Overall study design. The study utilizes patients from the MGB Biobank that were infected with SARS-CoV-2 at some point between the start of the pandemic (01/01/2020) through 10/27/2020 to generate the Longitudinal EMR and Omics COVID-19 Cohort (LEOCC). After exclusions, 592 patients were included, with 455 patients having plasma samples prior to COVID-19 infection, and 137 patients having one or more plasma samples during/after COVID-19 infection. <t>Metabolomic</t> profiles were generated on all plasma samples. We used profiling prior to COVID-19 infection to cluster individuals into three prospective metabolomic endotypes and assessed their relationship with both COVID-19 severity and influenza incidence/recurrence. We further evaluated the generalizability of these findings to metabolomic profiling during and after acute COVID-19 infection and evaluated the associations with COVID-19 severity and influenza infection/recurrence.

    Techniques:

Associations between COVID-19 severity and influenza infection with metabolomic endotypes . Chi-square tests determined the differences in clinical characteristics across metabolomic endotypes for categorical variables (COVID severity variables: severity, ICU admission, deceased; Influenza variable: Influenza existence) and one-way analysis of variance (ANOVA) tests for continuous variables (Influenza counts). Severity had four categories for a response after COVID-19 infection: No hospitalization, Inpatient/Emergency visit, ICU/Mechanical Ventilation/Intubation, and Deceased. The ICU Admission was a binary variable generated based on whether the patient had ICU admission or not: less severe (0) contained No hospitalization and Inpatient/Emergency visit, and more severe (1) contains ICU/Mechanical Ventilation/Intubation and Deceased. Influenza existence indicated whether individuals had Influenza (1) or not (0) from 01/01/2010, and Influenza counts indicated how many episodes of Influenza that individuals had from 01/01/2010. The upper layer ( a ) was for prior to COVID-19 metabolomic endotypes, while the lower layer ( b ) was for during/after COVID-19 metabolomic endotypes.
